Request a sick note for work

You do not require a doctors sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website
If you require a new sick note please submit a medical problem health form or book an appointment via reception. If you require a sick note extension you can request this below
If you are unable to use the Blinx online consultation tool to request a repeat sicknote then please contact the surgery via phone.
