Submit a Medical Problem form for any non-urgent medical problems you would contact your GP for. Our admins review and book health forms from the same day to the next working day.

We are limited in capacity for Medical Problem health forms and these will be switched off daily once we hit capacity, but will reopen from 8:00am the following day. 

You can use this form to request a copy of your test results, depending on the results and tests we may book an appointment for this, however often our admin team can send the results via text or email, many test results will now appear on your NHS app also.

You can use this form when you are requesting a Sick note Extension. If you would like to request a new Sick Note please use the Medical Problem Health Form.

If you need a letter from your GP, Please come into reception and we can give you a patient request form to complete. 

This form will allow you to submit to us your change of address, name or phone number, and will usually be updated the same day by our admin team.
